This paperwork is required by the school district through SNAP. It is tracked by the high school office and/or athletic office. It is your responsibility to complete this paperwork for your student to be eligible to participate in any extra-curricular activity.
All Marching Band / Color Guard, Concert and Jazz Band students will need to fill out the Google Form to ensure all communications are received.
